New Patient Registration

If you reside within the practice catchment area you can register with the practice.

The practice area consists of Penryn, Falmouth and all the villages and countryside on the map below.

Please register with us by coming to reception at any of our branches: Penryn (main site), Stithians or Mawnan.  Alternatively you may download registration forms to complete and bring in by clicking on the "Registration forms" tab above.

Non-English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

A GMS1 registration form can be downloaded from the link below.  To register at our surgery please print this off, fill it in and bring to reception.



When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and bring in at the time of registration.
