New Patient Registration

If you would like to register with the Practice as an NHS patient, please visit your nearest branch, either Penryn main site, Stithians or Mawnan Smith, to complete a NHS registration form and practice questionnaire. Information on how to do this can be found under the Registration Forms tab.

Alternatively register on-line via Campus Doctor via this link   This online service is available for ANY patient to register with Penryn Surgery - you do not have to be a student at one of the local universities to apply.

We may ask for proof of identity when you register, especially when you register children in your care. This may be used to check your details match with the information held on the NHS central patient registry and that your previous medical notes are passed on to us as your new practice.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered an initial consultation with the doctor. If you take regular medication please make an appointment with our Clinical Pharmacist well before you run out of your current supply.
